This form is for registering children for Junior Church for September 2024. You can register any Sunday at the reception desk, please leave a little extra time to do this.

This form is to be completed in full by a parent or carer only, and covers all regular attendance at St Mary's Junior Church on Sundays from September 2024. If you are a member and log-in, some of your information will have been pre populated.
Please let us know of any additional needs your child has, especially if they are receiving support in school.
